Attention parents of patients who are 18 + yrs of age:
All prior signed blanket permission slips that give parents access to your adult child’s medical record will no longer be in effect as of 3/13/18, our start date on EPIC (new electronic medical system).
Requests for information or medical advice will be handled on an individual basis with authorization from your adult child.
Any questions or concerns in regards to this change should be addressed with your child’s PCP.
